Community Pharmacy Patient Questionnaire - Online format
Each pharmacy has their own pharmacy specific page/s with the questionnaire/s hosted on the pharmacy-survey website, which you direct your patients to eg. http://www.pharmacy-survey.co.uk/your_pharmacy_group/branch.php .
You do not need your own pharmacy website to run an online questionnaire as participants can be sent directly to the pharmacy-survey webpage specific to your pharmacy via printed pamphlets/cards. If you already have a pharmacy webpage then a small paragraph about the questionnaire and a link to the pharmacy-survey webpage is all that is needed. If you do not have a webpage but wish to have a link to the questionnaire then the NPA offers a free webpage for its members about their pharmacy, where a link could be inserted to your questionnaire page.

For patients
- If you have a consulting room equipped with an on-line computer, patients could complete the questionnaire there and then.
- If the customer is in a hurry or you do not have patient accessible internet, then they can answer the questionnaire at home, at a library, or in an internet cafe etc at their convenience.
- If the customer is homebound and of limited dexterity, the internet may well be the preferred option.
- The online system are a popular option in busy pharmacies with little time or space.
For the pharmacy
- Cost effective.
- Reach a wide audience.
- Results ready in minutes.
- Easy to manage.
- Informed on the progress of the on-line questionnaire by email, and therefore will know when to stop recruiting patients in-store.

You should have finished the 2009/10 survey, and now starting to collate the results and starting to interpret them, with a view to sending them to your local PCT.
Being an essential service, participation is compulsory under the community pharmacy contract in England and Wales, but it is also a great opportunity to find out what your customers think of your services and staff and how it can be improved, it helps future business planning to improve key aspects of service, and ultimately may increase profitability.
If you have a pre-reg for the year, why not get them to conduct the survey and have them meet a lot of their competencies for the year.
